If something has happened, or something nearly happened, report it early so it can be reviewed and actioned properly.

Incident Report
Use this form to report an incident, injury, near miss, hazard or safety concern.
Reports should be submitted as soon as practical. Even small issues are worth logging, especially if they help prevent something more serious later.
For emergencies, call 000 first. For urgent site safety issues, contact your supervisor immediately.
Please note: Reports are shared with Sydney Relining and HM Focus, Sydney Relining’s safety consultant, so incidents and safety concerns can be reviewed and actioned. Where required, information may also be shared with relevant supervisors, contractors, insurers or regulators.

One central document covering responsibilities, consultation, incident reporting, emergency response, contractor expectations and general safety rules.
One consolidated SWMS pack for traffic interface, vehicle movement, loading/unloading, LPG, fuel, resin/hardener, liner inflation, restricted access and manual handling. SWMS are required for high-risk construction work and need to identify the work, hazards, risks and controls.
Lists hazardous chemicals and includes current SDS for resin, hardener, fuel, LPG and any other chemicals used or stored. SafeWork NSW says the register must include current SDS and be viewable by affected workers.
Covers 000, supervisor escalation, fire, chemical exposure, spills, vehicle incidents, notifiable incidents and who to contact. SafeWork NSW requires notification for deaths, serious injuries/illnesses and certain dangerous incidents.

These summaries are provided as a quick safety reference while detailed safety documents are being developed. They do not replace site-specific risk assessment, SWMS, supervisor instructions or worker judgement.
If the task cannot be completed safely, stop work and contact your supervisor.
High Risk
Traffic interface
Working adjacent to live traffic.
Critical controls
Set up a clear work zone. Keep workers separated from vehicle paths. Use cones, signage, barriers or spotters where required.
Stop work if
Traffic cannot be separated from workers, visibility is poor, or the work zone cannot be controlled safely.
High Risk
Liner inflation during wet-out
Splash and eye exposure.
Critical controls
Wear eye and skin protection. Keep unnecessary people clear. Control pressure carefully. Avoid standing in the line of potential splash, release or movement.
Stop work if
Pressure is not controlled, splash risk cannot be managed, PPE is not being worn, or people are too close to the work area.
